I have written that the microtubules and nucleus are disassembled during M-phase so that the chromosomes can be accessed by the microtubule. I am a bit confused; is it just that these microtubules are disassembled so that the spindle apparatus can form?

Correct! "Spindle apparatus" is just a fancy way of describing the "new" arrangement of microtubules emanating from each centrosome during mitosis. The cell has to get rid of the interphase microtubule array in order to build the spindle apparatus.
