How exactly is water formed from the ETC and proton pump? Are the electrons gathered from the ETC added to existing oxygens and do the hydrogens come from the proton pump?


Yes, the electrons come from the last carrier in the ETC. The oxygen is in the matrix of the mitochondria (and everywhere else in the cell) and using a human for an example, gets that oxygen to all its cells via the circulatory system. And finally, the protons (not actually full hydrogens) come from the matrix. It's important to recognize that although the ETC is pumping a lot of protons into the IMS, some of them are flowing back into the matrix via ATP synthase, so you're never going to "run out."
