I do not understand where the two other phosphate groups in our diagram of the incoming nucleotide are? Is their presence implied? Are they still bonded to the phosphate group participating in the phosphodiester linkage?

No, that pyrophosphate group is no longer part of the nucleotide, and in fact the pyrophosphate group will quickly be hydrolyzed into two inorganic phosphates (Pi).
