MCB 150 Conflict Final Exam Request

MCB 150 students: If you believe that you have one of the following situations and will require a conflict final exam, please fill out and submit this form. You must complete and submit this form by noon on Wednesday, August 1, 2007. Requests received after this deadline will not be considered. Provide as much information as possible according to the spaces provided, and click on the Submit Form button. Course staff will respond to your submission within 24 hours Monday through Friday. You will be notified via e-mail on Thursday, August 2, 2007 (Reading Day) before noon regarding the date, time and location of your conflict final exam. I understand that a conflict final exam for MCB 150 may be taken only if one of the following conditions is met. Check the box next to the situation that best fits your situation.

1. I have 3 final exams within a 24 hour period. (Code of Policies and Regulations Applying to All Students, Section 82.A.4)

2. I have another final exam scheduled at the same time as my MCB 150 final exam.

3. I have a valid excuse, and have received permission from a dean in my college to take the conflict final exam for this course. Written confirmation of this permission must be submitted to 252 Davenport Hall before 5 p.m. on Wednesday, May 2, 2007.

In order to schedule a conflict final exam for everyone who is unable to take their final exam at the scheduled time, we must have your complete final exam schedule. Please provide course numbers, dates and times of each of your final exams. The MCB 250 staff will send e-mail to you on Thursday, August 2, 2007 (Reading Day) to inform you of the conflict date and time. You are required to inform us whether or not you intend to take the conflict final exam by 8:00 a.m. on Friday, August 3, 2007—call 244-6238 or e-mail the course coordinator.
